`CodexExplored` groups file lookups under a bold `Explored` line. Each item starts with a blue verb, and searches can name a scope, as in `Search changelog|release in CHANGELOG.md`. Set `active` while calls are still running to show `Exploring` with a pulsing bullet. Extra calls in `details` stay folded behind `+ Show details`, a shadcn `Collapsible` trigger.
