CodexHeader renders the session banner: a blue >_, the bold product name with its version, the working directory under it, and a blue greeting line.

`cwd`	`~/acme/web`	The working directory under the name.
`greeting`	`Anything interesting on the docket?`	Blue line below the banner. Pass `null` to hide it.
